In the United States, COMTAN is indicated as an adjunct to carbidopa/levodopa therapy to treat patients with idiopathic Parkinson's disease (PD) who experience the signs and symptoms of end-of-dose "wearing-off." Wearing off is the reemergence of symptoms such as slowing of movement and increased stiffness before the next scheduled levodopa dose.

- COMTAN therapy can begin with the earliest signs of "wearing-off"
- COMTAN therapy should be added when:
- You are considering switching the carbidopa/levodopa dose from TID to QID
- You are considering fractionating the levodopa dose, or switching to a controlled-release formula
- The patient is experiencing "Off " symptoms upon waking in the morning
Due to the potential increase in dopaminergic side effects, the greatest clinical benefit may be achieved in those patients not yet experiencing dyskinesias.
